This means that your current provider may not be able to issue your medical certificate for CDL License renewal. Our providers are certified medical examiners that are qualified to issue your medical certificate.

Please bring the following items with you to your exam:

• Driver's license
• A list of prescriptions and over-the-counter medications (including the dosage and the frequency).
  
• A list of any medical conditions you may have.
  
• The name, address, and phone number of your prescribing health care providers and your local pharmacy.
  
• The name, address, and phone number of your local pharmacy.
  
• If you own or wear glasses or contacts, please bring them with you.
